How to structure it

Opening

Name the role and, in one line, the strongest reason you're a fit. Skip the throat-clearing.

Middle

One or two paragraphs of real, specific evidence: what you did, and the result. This is where most letters go generic. Yours shouldn't.

Why this employer

One genuine, specific reason you want this job at this company, not any company. Vague enthusiasm reads as a template that wasn't customised.

Close

Thank them, note your resume is attached, and make it easy to reach you.

Re: Application for Surgeon

I am writing to apply for the Surgeon position at [Company Name]. My training through the Royal Australasian College of Surgeons and experience managing complex operative and perioperative care make me well suited to this role.

In a recent case, imaging review ahead of surgery identified an unexpected anatomical variation that changed the planned approach. Adjusting technique in theatre while keeping the anaesthetic and nursing team informed throughout kept the procedure safe and the patient's recovery on track, and the case was later used as a teaching example in departmental peer review.

[add one genuine, specific reason: a project, value, or market position you can point to] is part of why I want to work at [Company Name] specifically, rather than sending a generic application.

Thank you for considering my application. I have attached my resume with further detail on my training, clinical experience and areas of surgical practice, and I would welcome the chance to discuss how I can contribute to your surgical team.

Getting it right

This is a starting point, not a guarantee of interviews. Make it specific to the actual employer before you send it.
